One more option I found when I used to repair these...


Boot into single user mode, set the drive as writeable using the command
"mount -uw /" (I think, the command will be written there), then run "rm
/var/db/.AppleSetupDone", reboot and it'll go through the annoying intro
movie and it'll let you set up a brand new admin account which will enable
you to erase or reset passwords to any account.

The .AppleSetupDone file is a plain text document that contains the
information that you put in during the setup process. When you remove it the
system thinks that there isn't an admin account and lets you create one.
