I've had no problem with SA; NC is usually what prompts people to want
to pay for a one time printing license of some kind.

Has anyone encountered a print publication or web publication that
thinks that SA results in compliance issues? eg: That the entire page or
publication being under the CC-SA-whatever? I haven't.

I've found that they just credit the photo, add a CC blurb of two or
three words, and move on.

Generally speaking, paying upfront is pretty much the norm. Or at least
the promise to pay and that promise is often forgotten.
