DPW Order 165,716. I contacted the sf department of public works
before starting this project, they told me the following:

Thanks for your e-mail; this would most likely require a Temporary
occupancy Permit at $55.30/day + a $4.50 surcharge.

Our basic guidelines, since this would involve a booth set-up on the
sidewalk and as I understand it, open to the general public, it would
need to be both accessible to the disabled and blind, as well as
providing a minimum 6' clear path of travel to pedestrians.

Attached for your information are copies of our Application and
general guidelines for a Temporary Occupancy Permit; upon review of
this information, if you have any further questions, please do not
hesitate to contact me via e-mail or by phone at (415) 554-6199.
Please be advised that we will also require the submittal of a
Certificate of Insurance per the attached sample.

Regards,

Nick Elsner
